Abstract
A 68-year-old man was admitted to the emergency department with acute retrosternal chest pain, nausea, and diaphoresis. The initial 12-lead ECG revealed ST-segment elevation in leads II, III, and aVF with reciprocal ST depression in leads I and aVL. Because of the clear STEMI pattern, immediate primary percutaneous coronary intervention was performed. Coronary angiography demonstrated an acute thrombotic occlusion of the right coronary artery (RCA). Successful percutaneous transluminal coronary angioplasty with implantation of a drug-eluting stent was achieved. On the first day after the intervention, transthoracic echocardiography performed in the department of functional diagnostics demonstrated inferior wall motion abnormalities with mildly reduced left ventricular systolic function. During further hospitalization, the patient developed a symptomatic second-degree atrioventricular block (Mobitz type II), requiring temporary pacing followed by implantation of a permanent dual-chamber pacemaker.
